Mrs. Cook passed away May 23, 2025, at Mercy Hospital in Ada.  She was born February 23, 1959, in Morris, OK to Kenneth and Kathryn Sapp Smith.

On July 12, 1996, she married Stephen Ray Cook.

Survivors include her husband, Stephen; son, Ernest Frisby II and wife Mandy; daughter, Kathryn Walter and fiancé, Nick Slezak; brothers, Timothy Smith, Donald Smith, and Michael Smith; sister, Debra Leremy; and grandchildren, Zoey Forsyth and husband Kyle and Lyla Grace Slezak.

She was preceded in death by her parents; brothers, Randy Smith, Gerald Smith, and Larry Smith; and sister, Sondra Carter.
