ADA-Graveside services for Carolyn Diane O'Neal Jack, 72, of Ada, are 3:00 p.m. Thursday at Oaklawn Cemetery in Wynnewood, Rev. Hubert Conway will officiate.
Mrs. Jack raptured Sunday, June 28, 2015 at an Ada hospital. She was born February 2, 1943 in San Francisco, CA to Johnny Bill and Mary Adelia Goodner O'Neal. She attended grade school in Arizona and graduated from high school in Morenci, AZ. She attended Grand Canyon University and received a Dental Hygienist degree in San Francisco.
She married Billy Bose Jack, Jr. October 17, 1964 in Morenci, AZ. Mrs. Jack had been a dental hygienist and later, she was employed with the Ada Schools in the Cafeteria for 18 years. She was a member of the First Baptist Church in Ada.
Survivors include her husband, Billy, of the home; three sons, Tyler Jack and wife Marcia, Troy Jack and wife Julia, and Billy Jack III, all of Ada; a sister, Vicky Lynn Hedrick and husband Randy, Wynnewood; her grandchildren, Taryn Nicole Jack, Tanner Kane Jack, Barrick O'Neal Jack, Natalie Elizabeth Jack and Colton Thomas Jack; and a daughter-in-law, Cheri Jack.
She was preceded in death by her parents and a brother, Kenny O'Neal.
The family says those who wish may make memorials to the First Baptist Church Falls Creek Building Fund, 521 S. Broadway, Ada, OK 74820.
